Dr Ana Sierra de Aragon accepts most major insurances; however, this is an ever changing process to keep it current on this site; as such, please validate with your insurance proper coverage.

If you have any questions about coverage, please, contact your insurer prior to scheduling an appointment to validate that Ana Sierra de Aragon MD participates in your plan.

You are expected to understand the plan your contracted with your insurance companies.

Patients are asked to pay their insurance co-payment at the time of their visit.

Patients are asked to pay their deductible, if any, in addition to their copay, at the time of their visit.

Any outstanding balance is due within 10 days.

If you do not see your insurance on our In-Network list, please, do contact your insurance company.

Please, always bring your insurance card and identification.

We provide large time slots for new patients, so if they do not show up, we may no longer offer them an appointment. They must cancel or reschedule at least 1 business day in advance

All refills require an appointment for safety reasons

  1. Please, plan accordingly.
  2. Request them during your visit; otherwise, a new appointment is required; especially for controlled medications.

Up to two of your symptoms may be addressed per appointment. Please, plan accordingly.

Follow up visits may be required.

Prepare for your appointment by having questions written down, list of medications you are taking, insurance card, identification, address, phone and email up to date.

If discharged from hospital, visit us in the first 3 to 5 days.

Provide accurate insurance information so that we can bill on your behalf; if inaccurate, patient is financially responsible.

  1. If your Insurance Company requires pre-authorization and special handling:
    1. It may take up to 10 business days to complete their process.
    2. We will provide up to two of such referrals per visit.
  2. Medical Assistant will contact you when your Referral is completed.
  1. Cancellations must be made at least one business day in advance to avoid $50.00 fee
  2. Same day cancellations incur a fee of $50.00
  3. A fee of $50.00 is charged if you do not attend your appointment

If you have a complaint on the service we provided to you or failed to provide; we will greatly appreciate if you let us know and give us an opportunity to review it and fix it.

Patients with HMO and AHCCCS plans, please, you must call your insurance company and update your PCP prior to your appointment. Failure to do so will mean delays in seeing you and you having to pay out of pocket for your visit.
