Prepare for your appointment

Due to Insurance changes, up to TWO of your symptoms may be addressed per appointment. Please, plan accordingly.

Follow up visits may be required.

Prepare for your appointment by having questions written down, list of medications you are taking, insurance card, identification, address, phone and email up to date.

If discharged from hospital, visit us in the first 3 to 5 days.

Provide accurate insurance information so that we can bill on your behalf; if inaccurate, patient is financially responsible.

Prescription Refills

Processing Refills may take up to 1 week.

    1. Please, plan accordingly.
    2. Request them during your visit; otherwise, a new appointment is required; especially for controlled medications.
    3. Future refill requests must be sent by your pharmacy electronically.

Referrals to Specialist

  1. If your Insurance Company requires pre-authorization and special handling:
    1. It may take up to 10 business days to complete their process.
    2. We will provide up to TWO of such referrals per visit.
  2. Medical Assistant will contact you when your Referral is completed.

Cancellation of Appointments

    1. Cancellations must be made at least one business day in advance.
    2. Same day cancellations incur a fee of $50.00
    3. A fee of $50.00 is charged if you do not attend your appointment
